If you leave Germany, you don't have to lose everything you've paid into PHI. The German market offers a specific mechanism — Anwartschaft — that lets you keep your entry age and ageing reserves in case you return.

Cancellation

You may cancel PHI when you deregister (Abmeldung) from Germany. Bring the Abmeldung certificate to your insurer. Your ageing reserves are typically lost.

Entitlement (Anwartschaft)

For €30–€80/month you can freeze your PHI contract. When you return to Germany, you resume the policy with your original entry age and preserved ageing reserves — no new health check.

When Anwartschaft is worth it

If there's any realistic chance you'll be back in Germany within 10 years and your health may change, Anwartschaft is one of the smartest bets in the German insurance market.
