The switch from GKV to PHI is a one-way door for most expats — and one you rarely regret if you time it correctly. Here are the rules.

When you're allowed to switch

  • As an employee above the JAEG threshold — after crossing the threshold for the required period.
  • As a self-employed professional — at any time.
  • When starting civil service — from day one.

Notice periods

GKV requires 2 months' notice to the end of a month once you've been insured at least 18 months, or immediately when your circumstances change (e.g. becoming self-employed).

The age-55 trap

Coming back from PHI to GKV after age 55 is legally almost impossible. Every PHI decision should be planned as a long-term commitment — which is exactly why choosing the right tariff at entry matters so much.
